To apply for DBL, the claimant needs to file form DB 450 with the state or with their insurance carrier. If the employer writes benefits through the New York State Insurance Fund, claimants should complete form NYSIF DB 450 and submit the claim to the state insurance department.
If your client writes disability benefits or enriched DBL through a private carrier, you can access the appropriate form DB 450 through your Broker Dashboard, and provide the correct form to your client based on their carrier. The DBL Center can help you file the claim.

What benefits do I qualify for?
Anyone in New York who works for a covered employer and suffers a non-work-related illness or injury may qualify for DBL insurance. Full-time employees qualify after they have completed four consecutive weeks of employment (for that employer or another covered employer in New York). Part-time employees may qualify after the 25th day of regular employment, unless their eligibility was already established with another employer.
Disability benefits cover non-work-related illnesses, injuries, pregnancies, and childbirth. Workers who don’t qualify for DBL may qualify for PFL in NY. PFL allows for up to 12 weeks off at partial pay following the birth, adoption, or foster care of a child within the first year of an event, to care for a loved one who is seriously ill or injured, or to take care of things at home while a military family member is deployed for active duty.
How much does NY disability pay?
NY short-term disability, under the Disability Benefits Law, pays 50% of an employee’s salary, up to a maximum of $170 per week for up to 26 weeks.
When will I receive my payments?
There is a 7-day waiting period for all short-term disability claims in New York. You will receive your first payment on the eighth day after filing your claim using the short-term disability form.
